What does Google say about Google Voice and HIPAA?

Check your visibility →Google lists Google Voice as covered functionality with one condition. Its HIPAA Included Functionality list, dated August 31, 2026, names Google Voice for "managed users only." Google's HIPAA Implementation Guide, dated September 2025, adds two sentences:
- "Licensed users of Google Voice are covered under the Google Workspace BAA."
- "Administrators should obtain Google Voice licenses for users that handle PHI."
Two conditions follow. The user must be managed by the organization's Workspace administrator. The user must hold a Google Voice license.
How do you check which Google Voice account you have?
Open Google Voice settings and read the account line. Google's help page gives the test. If you see "This account is managed by your Google Workspace administrator," you have a Voice for Google Workspace account. If you see no managed-account message, Google says you have a personal account or a work account that your organization does not manage.
A free number on a personal Gmail is not covered
Many solo clinicians use a free Google Voice number as a practice line. That account has no BAA. Do not use it for patient calls, texts, or voicemail.
Which Google Voice plan does a practice need?
A practice needs a Voice plan added to a Google Workspace account. Google's product page lists three plans.
| Price per user per month | Notable features | |
|---|---|---|
| Starter | $10 | Up to 10 users as a Workspace add-on, voicemail transcription |
| Standard | $20 | Unlimited users, auto attendants, ring groups, on-demand call recording |
| Premier | $30 | Automatic call recording, advanced reporting |
Google also sells a standalone Starter plan for one user without Workspace. Google's HIPAA documents do not say whether that plan counts as a managed account. Ask Google in writing before you use it with patients.
How do you set up Google Voice for patient calls?
Accept the BAA
A Workspace administrator accepts Google's BAA in the Admin console. Save a dated record.
Add Voice to the Workspace account
Buy a Voice plan as an add-on to the practice's Workspace subscription.
Assign a license to each user
Give a Voice license to every staff member who handles patient calls, texts, or voicemail.
Move personal numbers
Migrate any clinician who uses a personal Google Voice number to the managed account. Google documents this migration.
Set calendar entries to Private
Google's guide recommends this for entries with patient information, because a Voice user dialed into a Meet call can see the meeting name.
Turn on 2-Step Verification
Require it for every account that can read voicemail or texts.
Are Google Voice texts and voicemail covered?
Google's HIPAA Implementation Guide does not address text messages, voicemail transcription, or call recording in Google Voice. It covers the service as a whole for licensed, managed users. A practice should decide its own rules for three features:
- Texts. Keep clinical details out of texts. Use them for scheduling.
- Voicemail by email. Send voicemail transcripts only to mailboxes inside the same Workspace account.
- Recording. A recorded patient call is patient information. Record only with a reason and a storage plan.
Our Google Workspace HIPAA guide covers the BAA and the other covered services.
