No. A medical website does not need an llms.txt file to appear in Google Search or in Google's AI features. Google says it does not use the file. The file is optional, cheap to make and harmless. We checked Google's and Chrome's own documentation on October 15, 2026.

What is llms.txt?
Is AI recommending you?
Webpatch can show you how your practice appears in ChatGPT, Google’s AI, and search — and what to fix first.
Check your visibility →It is a Markdown file that summarizes a website for AI tools. The llmstxt.org site calls it "a proposal to standardise on using an /llms.txt file to provide information to help agents use a website." Jeremy Howard published it on September 3, 2024. The site says the specification "is open for community input."
The format is simple. Only one part is required: "An H1 with the name of the project or site." A short summary in a blockquote usually follows. Then H2 sections list links to key pages.
Does Google use llms.txt?
No. Google's AI optimization guide, last updated July 10, 2026, says: "You don't need to create new machine readable files, AI text files, markup, or Markdown to appear in Google Search, as Google Search itself doesn't use them."
The same page adds that making the file is fine. Google says doing so "will neither harm nor help your site's visibility or rankings in Google Search."
Why does Chrome Lighthouse check for llms.txt?
Lighthouse is Chrome's site audit tool. Its llms.txt audit page calls the file "an emerging convention used to provide a machine-readable summary of a website's content." It flags a page only when the file returns a server error. A missing file is marked N/A. The page says "providing the file is optional at the moment."
Two Google teams, two messages
Google Search says it ignores llms.txt. Chrome's Lighthouse audits it for AI agents that browse the web. Both say the file is optional. Neither says it improves rankings.
Is llms.txt the same as robots.txt?
No. They do different jobs. Google's robots.txt guide says: "A robots.txt file tells search engine crawlers which URLs the crawler can access on your site." llms.txt does not grant or block access. It only points AI tools to pages you think matter.
Neither file protects private pages. Google says robots.txt "is not a mechanism for keeping a web page out of Google." Patient portals and intake forms need real access controls, such as logins. Do not rely on either text file to hide them.
Will llms.txt help your SEO?
Not in Google. People ask this often: Ahrefs shows about 400 US searches a month for "will llms.txt file help your seo" (checked October 15, 2026). Google's answer is the quote above. We could not confirm from primary sources in this run whether ChatGPT, Claude, Perplexity or Bing read the file. Treat any claim that they reward it as unproven.
What should a medical website do instead?
Fix the basics Google names. Its guide says "a page must be indexed and eligible to be shown in Google Search with a snippet" to appear in AI features. On structured data, it says: "Structured data isn't required for generative AI search," but "it's a good idea to continue using it."
Indexable pages
Check that service, provider and location pages are indexed and not blocked by robots rules or noindex tags.
Direct answers
Open each page with a one-sentence answer to the question patients ask.
Structured data
Keep accurate MedicalClinic, Physician and FAQ markup where it fits.
Clean tracking
Keep third-party pixels off pages that collect health details.

Should you create an llms.txt file anyway?
It is a reasonable low-cost step once the basics are done. A small practice site needs only a short file. Follow these steps if you make one.
Create a Markdown file
Name it llms.txt. Start with an H1 that holds the practice name.
Add a short summary
Write one blockquote line: specialty, city and who you treat.
List public pages only
Add H2 sections with links to services, providers, locations and contact. Leave out portal, login and form pages.
Upload to the site root
llmstxt.org says the file can sit at the root or under a path, covering pages under that path.
Check the response
Open the URL and confirm it loads without a server error, since that is what Lighthouse flags.
What does an llms.txt file look like for a practice?
It is a few lines of plain Markdown. The table shows the parts the llmstxt.org proposal describes, with an example line for a fictional clinic.
| Required? | Example line | |
|---|---|---|
| H1 with the site name | Yes, the only required part | # Example Family Dental |
| Blockquote summary | No | > General and cosmetic dentistry in Springfield for adults and children. |
| H2 sections with links | No | ## Services, then a list of links to service pages |
| Optional section | No | ## Optional, for secondary links an agent can skip |
Keep the file in step with the site. If a service page moves, update the link. A file full of broken links is worse than no file.
